Leírás

Hazel Hall Nursing Home is a leading and well established residential, convalescent and day care centre for older persons and vulnerable adults.
Hazel Hall Nursing Home is a leading and well established residential, convalescent and day care centre for older persons and vulnerable adults aged 18 years and over.
Hazel Hall Nursing Home is registered with and governed by the Health Information and Quality Authority (HIQA). It is a service provider under the Nursing Homes Support (Fair Deal) Scheme and is approved by VHI Healthcare and other healthcare schemes for members who require a period of convalescence following a hospital stay.
At Hazel Hall Nursing Home, we are registered to provide care and services to a maximum of 46 persons and accommodation comprises 44 single rooms and 1 twin bedroom.
We have a team of 70 staff providing care and services to each person living at Hazel Hall and this ensures we know the people we care for well, which is a key priority for us in providing a home from home situation for the persons living here and for providing truly person-centred care.
To provide you with a flavour of our ethos at Hazel Hall Nursing Home, I will set out our mission statement.
Hazel Hall Nursing Home is not only renowned for its clinical and medical care, but also for the priority it places on improving the quality of life of the person and the promotion of the personal, social, psycho-social and emotional well being of people who live at Hazel Hall.
Following the person's admission, a key priority for our team in improving the person's quality of life and well being is to 'get to know' the person and to gain an understanding of their core values, the things that are important and meaningful to them, the feelings they currently have in their life, what they do in their life and what they want to do in their life, their strengths and talents in achieving what they want to do in their life, and the requirements for support.
Our Well Being Manager and her team provides persons living at Hazel Hall with social support and opportunities for meaningful engagement in social activities, leisure interests and hobbies, while a Complementary Health and Well Being Therapist concentrates on and provides support on the emotional aspect of the person's life. An in-house Advocate ensures that each person's rights are upheld and that the person is facilitated to live in dignity.
As outlined on our Mission Statement, we are committed to listening to, and working together with, individual persons who reside at Hazel Hall, their families, representatives, other professionals and authorities in providing for our residents holistic, person-centred care and promoting a high quality of life and well being, respectful of the individuals rights and wishes in an environment where each resident and employee matters and is safe, supported, valued and enabled'.
Persons living at Hazel Hall Nursing Home have an inherent set of rights and are provided with an opportunity to choose with whom their information is shared and whom from the family should be involved in the care process.
There are times when person-centred care cannot happen on its own and the involvement of family members is integral to the optimal care of the person. Examples of these circumstances would be where the person has memory problems or where the person is living with a dementia. This is where relationship-centred care begins. Relationship-centred care involves staff getting together with the person and, with the permission of the person, with family members. Family members are seen as care partners, participating positively in the person's care.
When a loved one moves into long term care, family members can experience a wide range of emotions, from guilt, to relief, to feelings of loss, to feelings of anticipation that the same level and quality of care will be provided to their loved one as had been provided at home. These are common and perfectly natural emotions and are deserving of recognition.
Although no longer physically responsible for the day to day care of a loved one, families are encouraged to share their skills and expertise in knowing their loved one best. Families can provide useful information to staff in terms of the care interventions and activities that have worked in the past and families are encouraged to work with staff to develop the person's care plan and review it together regularly.
Families can bring in meaningful photographs and compile, or assist in compiling, the person's life story book. Objects of significance from the person's home can also be brought in by family members to be placed in the person's bedroom or memory box.
Families can become involved in continuing the person's links with the home and local community by organising visits from others, planning an outing, perhaps a short break - or longer, if the circumstances allow.
Families generally soon recognise that time spent with their loved one is better quality time and this can often strengthen relationships and lead to the optimal care and well being that the person deserves.
ADMISSIONS:
Admission to Hazel Hall Nursing Home is arranged by appointment following a pre-admission assessment of the person’s needs. This is to ensure that we have all the necessary equipment, knowledge and competency to meet the person’s care needs. Those requiring care at Hazel Hall, along with their families, are invited to come and meet our team and have a tour of Hazel Hall prior to admission to ensure that Hazel Hall 'feels right' to them in advance of any decision-making.
We understand that in exceptional cases, where there is no alternative available, emergency admissions are necessary to promote the safety of the person. In this circumstance, we require the following information in order to commence the assessment and care planning process:
  • Multi-Disciplinary Care Plan
  • Signed prescription
  • Current healthcare plan membership number
  • Current GP and medical card details
  • Consent where necessary
Details of any future medical appointments in relation to the ongoing care of the prospective Resident, i.e. any of the following may apply:
  • Gerontology Assessment
  • Warfarin Assessment
  • Palliative Care
  • Pain Management Care
  • Psychiatry of Old Age Care
  • Psychology Care
  • Oncology Care

ASSESSMENT:
The Person in Charge will arrange a comprehensive assessment, by an appropriate health care professional, of the person health, personal and social care needs immediately before or on admission to Hazel Hall Nursing Home.
Each individual will be central to their assessment and comprehensive care (i.e. physical, mental, psychological, social, spiritual, etc.) will be provided. Nursing care is based on the assessed needs of the individual and informed choices of the person and may require the interventions of other integrated multidisciplinary services.
CARE PLAN:
A care plan will be developed with the person's full involvement and participation following admission. This will be personalised and will set out the person's personal care goals, strengths, support requirements and agreed care interventions.
The care plan will be reviewed following the person's feedback, any changes to personal needs and circumstances and will be updated no less frequently than at four monthly intervals in consultation with the person, and where appropriate, with the person's family.
It is important to remember that the person is the owner of their healthcare record. As such, the person is entitled to access their healthcare record and will be made available to the person, and may, with the person's consent, or where the Person in Charge considers it appropriate, be made available to the person's family.
Hazel Hall Nursing Home is purpose built, and set in its own secure grounds with car parking facilities and is monitored by CCTV. It contains 45 bedrooms (44 private and 1 semi-private rooms). Each room is equipped with Cable TV (Flat Screen) and call bell system.
On entering Hazel Hall Nursing Home, reception and the office of the Person in Charge is to the right and the clinical treatment room is to the left.
