Hafod-Y-Green is the specialist rehabilitation division of a family owned healthcare service established in 1990.
It is located at Trefnant, near to St. Asaph in North Wales, and is situated in three acres of woodlands and gardens.
Beautiful Location
Hafod y Green is a specialist rehabilitation and nursing centre, set in three acres of woodland and gardens. The Centre is located in the beautifully picturesque Vale of Clwyd near to St. Asaph in North Wales.
Nursing Services
The Centre provides a high quality of specialist rehabilitation and nursing service for those with acquired head injury and other severe physical disabilities such as spinal injury and stroke.
Specialist Care
In recognition of disabilities such as Hunttingtons, Multiple Sclerosis and Parkinson’s disease the Centre provides a high quality of specialist nursing care.
Through intensive therapeutic intervention, the Centre will assist patients to relearn how to move again, communicate, develop reasoning and memory strategies, to gain personal living skills, and support patients to come to terms with their disability.
The skills and facilities at Hafod y Green are totally dedicated to achievement of these aims.
The aims of the Centre are to:
Provide effective individual programmes designed to Maximise recovery of physical, cognitive and behavioural Functions leading to the best possible quality of life.
Provide training and support to the carers and families of patients.
Facilitate early and successful return of the patient to the family and community.
Promote and encourage independence and dignity to enable each patient to maintain a high quality of life irrespective of their disability and illness.

STANDARDS AND QUALITY
The Centre is registered with the Care and Social Services Inspectorate (CSSIW) for the physically disabled. For each resident, clear outcomes are defined and measured by standards, taking into account their quality of life.
To assist in achieving our aims, we have developed and operate a quality management system which has gained BS EN ISO 9000: 1994 Certification, including aspects specific to the care and nursing professions.
MISSION STATEMENT
It is our intention to assist and encourage people with disabilities in all aspects of their daily life, regardless of their race, creed or colour, by providing the facilities necessary for their mental, physical and spiritual well-being.
Each person, whatever his or her disability has the right to be treated with respect, dignity and above all as a valuable individual who has a role to play in the community. Therefore, it is our belief that people with disabilities should have the freedom to pursue their aims in life.
Facilities & Services
Treatment methods at Hafod-Y-Green are based on intensive rehabilitation by a dedicated interdisciplinary team incorporating the following skills:
  • Psychology
  • Rehabilitation Nursing
  • Counselling
  • Physiotherapy
  • Speech Therapy
  • Occupational Therapy
  • Medical Supervision and Nursing
  • Specialist Support Groups
In addition to intensive interdisciplinary in-patient rehabilitation, the Centre also provides long term and respite care and an advisory service.
To promote a rehabilitation programme that is transferable to the community setting, close liaison is maintained with general practitioners, social work professionals, solicitors, support agencies and the families and friends of residents
The facilities at Hafod-Y-Green consist of art and craft rooms, activities centre, pottery and ceramics room, speech and occupational therapy rooms, interview and discussion rooms, computer room, hairdressing salon and resident’s kitchen, workshops and a multi-sensory room.
In addition Hafod-Y-Green has a large garden with outbuildings and greenhouses and an additional multi-sensory garden which form part of the Centre’s therapeutic activities.
PHYSIOTHERAPY
The Homes physiotherapist has experience in managing a neurological rehabilitation unit, previous to this, she worked for 8 years in the intensive care unit at the local hospital developing a specific interest in non-invasive ventilation and running the respiratory team.
Individual rehabilitation programs are centred on mutual realistic goals, achieved through collaboration with the multi disciplinary team, treating each individual with respect and dignity and providing training and support for the carers and the family.
REFERRAL AND ADMISSION
The Centre will accept referrals from all proper authorities including hospitals, health authorities, social services, solicitors, support agencies and of course the families.
Where a referral is considered suitable, a visit to the Centre will be offered to the potential resident and family in order to discuss their needs with specialist staff. Prior to entry, the Centre should receive copies of all the medical, therapy and nursing reports available for the patient at the time of admission.
For those who make the decision to give up one’s home for residential care a trial period is recommended as the best way to reach the right decision. It is our intention to offer to the resident and family our full support to ensure that the transition from home to Hafod-y-Green is as smooth as possible.
Whilst at Hafod-y-Green it is our aim to hold regular reviews of the service we provide to each resident with all associated professionals and family. This will ensure that a high quality of service is maintained and where possible, all needs catered for.
